Republicans Target Medicaid Fraud Amid Allegations of Widespread Improper Payments Under Democratic Leadership

WASHINGTON, D.C. — As the nation approaches a critical juncture in Medicaid oversight, Republican leaders, including former President Donald Trump, are intensifying efforts to combat what they describe as rampant fraud in the program — a crisis they attribute largely to Democratic policies that have allowed billions in improper payments to proliferate.

Medicaid, the federal-state program providing health coverage to millions of low-income Americans, has long struggled with improper spending. However, recent analyses suggest that the scale of waste and fraud has escalated dramatically since the expansion of Medicaid under the Affordable Care Act (ACA). According to a report highlighted by Fox News commentator Michael Greibrok, Democrats have “supercharged” the problem by expanding eligibility to tens of millions of able-bodied adults without rigorous verification, allowing fraud to flourish unchecked.

During the Obama administration, the ACA’s expansion significantly increased Medicaid enrollment, but the administration reportedly declined to enforce strict eligibility checks. This lax approach continued under the Biden administration, which critics say has deliberately obscured the growing levels of waste and abuse. Meanwhile, many states have been accused of neglecting their responsibility to police Medicaid spending, relying instead on federal “good faith waivers” that have deferred accountability.

The consequences are staggering. The Trump administration’s 2020 audit found that 27.4% of federal Medicaid spending was improper, amounting to approximately $120 billion in erroneous payments. Most of these improper payments stemmed from eligibility errors, diverting funds from those genuinely in need. Experts estimate that at least one in five Medicaid dollars remains misspent today.

Compounding the issue, states face a federal mandate to reimburse the government if improper payment rates exceed 3%. With error rates far surpassing this threshold, states have so far avoided penalties only because of temporary waivers. However, beginning in 2030, states will be required to repay the federal share of improper Medicaid payments, a prospect that has alarmed officials nationwide.

Republicans have responded by pushing legislation signed by President Trump on July 4, 2025, aimed at tightening eligibility verification and eliminating waivers that have allowed states to delay reforms. The law empowers federal agencies to conduct more rigorous audits and holds states accountable for fraud and abuse.

These efforts have gained traction amid high-profile cases such as the Minnesota Medicaid fraud scandal, where investigations revealed that potentially half of $18 billion in federal funds intended for state-run programs may have been stolen. Minnesota’s case is emblematic of a broader national problem that experts warn could cost taxpayers trillions over the next decade.

The Department of Health and Human Services (HHS) oversees Medicaid and has pledged to increase oversight and enforcement. Meanwhile, the HHS Office of Inspector General continues to investigate improper payments and fraud schemes across multiple states.

Republican lawmakers argue that restoring integrity to Medicaid is essential not only to protect taxpayer dollars but also to ensure that vulnerable populations receive the care they deserve. They contend that Democratic administrations have prioritized political expediency over program integrity, allowing fraud to flourish at the expense of honest beneficiaries.

As states prepare for the financial repercussions of improper payments, the federal government’s renewed focus on Medicaid fraud signals a turning point. The coming years will test whether these reforms can effectively curb waste and restore public trust in one of the nation’s largest social safety net programs.
